Good Faith Estimate for Services

Centre Chiropractic, PC, is committed to transparent pricing. Under the No Surprises Act, uninsured or “self-pay” clients are entitled to a Good Faith Estimate of expected costs for chiropractic services, such as exams, adjustments, or therapies.

Provide us, by email or phone, with your contact details and requested services. We’ll provide a written GFE within 1–3 business days which will list typical services and costs. Your fees will depend on your treatment plan. Estimates are not a contract and may adjust based on clinical needs.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like radiographs, diagnostic tests, or durable medical equipment.
  • You can ask any healthcare provider for a GFE before you schedule. They are required to do so in writing at least one business day prior to your visit.
  • If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
  • Make sure to save a copy or take a picture of your GFE.

  • No Guarantee of Outcomes: Chiropractic care is provided based on professional standards, but we do not guarantee specific health outcomes. Treatment plans are individualized and discussed during consultations
  • Billing and Payments: You are responsible for all charges for services rendered. See our About Centre Chiropractic page for a list of accepted insurance plans. Insurance claims are submitted on your behalf, but you are liable for any non-covered costs
